Blue Cross and Blue Shield of Montana (BCBSMT) is seeking a Registered Nurse with a current, unrestricted Montana Nursing License to join our team as a Care Coordinator in our Helena office. As a Care Coordinator, you would be responsible for plans of care for members with medical and surgical healthcare needs, disease management interventions, and liaison services between patient, family, and healthcare personnel. The Care Coordinator promotes necessary medical care, provides patient education, and encourages appropriate utilization of BCBSMT networks and member benefits. Supported by a team environment in our Helena, Montana office, the Care Coordinator will provide superior service to all internal and external customers.
Hours are M-F full time, with weekends and holidays off! To qualify, you must have three yearsÂ' of recent direct health care experience, RN with unrestricted Montana nursing license, and current CCM accreditation (or ability to sit for exam within 2 years of hire.)
