DUTIES:
The Clinical Nurse Leader (CNL) is responsible and accountable for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self management after discharge and supporting factors that impact customer satisfaction of inpatients as assigned. The CNL possesses the knowledge and skills to effectively apply all aspects of the nursing process and care management principles within a collaborative, interdisciplinary practice setting, which will then enable effective implementation of health promotion and prevention practices, management of acute and chronic illnesses, attainment of appropriate lengths of stay and most effective level of care. This includes understanding changes associated with aging and principles of growth and development relevant to the adult and geriatric population. Core functions include daily assessment of patient care needs to determine the most appropriate use of expert! ise in clinical coordination, patient and staff education, attention to! systems issues that require intervention and quality and performance measures.
Many of these functions require ongoing daily collaboration with assigned staff nurses, the nurse manager and medical teams.
He/She identifies and assists with identifying education needs and customer service issues. The CNL will assist the nursing staff in the provision of direct patient care in complex situations. He/She participates in assigned medical center committees and provides education throughout the Medical Center on the CNL role.
He/She interacts with the hospitalist as required to meet role responsibilities.
The CNL collaborates with others in evidence based practice activities to improve care, including assessment, implementation and evaluation of care.
As a Coordinator, the nurse will:

Improve patient outcomes by utilizing performance measures to ensure quality of care
·
Review all clinical remi! nders and performance measures on a regular basis and assist providers with ordering appropriate labs, tests and exams
·
Monitor and evaluate unit practice and application of policies and procedures, ensuring standards of care and standards of nursing practice are followed
·
Support and execute ongoing competency, staff development and orientation process in coordination with the Education Program Coordinator and Supervisor
·
Work with the interdisciplinary team to promote integrated care for the veterans
·
Assist in developing, implementing, and evaluating primary care and interdisciplinary projects and programs related the primary care clinic
·
Support a global perspective in prioritizing work and assisting team function
·
Identify, implement and evaluate processes to increase patient satisfaction and plan of care
·
Assist with case manageme! nt for high risk veterans, and their individualized plan of care
·
Will follow up monthly on positive fecal occult blood tests to assure that the provider has addressed these
·
Assist with tracking and follow up care for patients that have Â"no showedÂ" primary care appointments

QUALIFICATIONS REQUIRED:
Citizen of the United States. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency
and
accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territor! y or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia
. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
Must be proficient in spoken and written English

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, no experience.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of experience, or an ADN or Diploma in Nursing and a bachelor's degree in a related field and no experience; or a Bachelors of Science in Nursing (BSN) and no experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of experience, or an ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 1-2 years of experience; or a BSN with approximately 1-2 years of experience, or a Master's degree in nursing (MSN) or related field with a BSN and no experience.
Nurse II - A BSN with approxi! mately 2-3 years of experience, or ADN or Diploma in Nursing and a bachelors degree in a related field and approximately 2-3 years experience or a Master's degree in nursing or related field with a BSN and approximately 1-2 years experience, or a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no experience.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years experience or Doctoral degree in nursing or related field and approximately 2-3 years experience.
