Beartooth Healthcare Community is seeking an experienced RN for our Transitional Care Unit to care for patients who have a goal to return home after hospitalization.
Primary Responsibilities:
The Transitional Care Nurse (TCN) is an expert in providing comprehensive care to chronically ill patients as they transition from hospital to home. We offer a Primary Teaming Model of Nursing on our TCU, this means you will work directly with a CNA to meet the care needs of patients. You will further develop your skills in areas such as Infusion Therapy, Tracheostomy Care and Wound Care. You will love watching patients get well and! return home.
You will work closely with the Director! of Nursing to help develop this new unit.
Qualifications:
Required: Experience in a skilled Nursing Facility Rehab Unit, LTAC, or Inpatient Rehabilitation Facility.
Current RN license.
