Job Title: Registered Nurse
Department: RiverStone Health Hospice Home
Reports To: RN Supervisor
FLSA Status: Non-Exempt
Work Hours: Full time, Nights, 7 PM to 7 AM 3 days per week
Hourly: $ 23.00 to $ 36.00 per hour, dependin g on number of years of transferrable experience and internal equity.

SUMMARY: The hospice nurse is responsible for the delivery of quality patient care through the nursing process of assessment, nursing diagnosing, planning, implementation, and evaluation. The nurse is responsible and oversees and directs patient care, documentation and customer satisfaction of all patient care services. Duties include assessing the patient's palliative care needs, determining level of care, working with the patient, family and interdisciplinary team to develop the plan of care, and assist teammates with duties of caring for the hospice home. The hospice patient can include those with cancer, chronic illnesses, accidents, disabilities or cognitive impairments at end of life.
ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES include the following. Other duties may be assigned.
1. Ability t o work collaboratively with the team to provide and end of life care and instruction for patients and families based on established clinical nursing practice standards.
This includes but not limited to:
? Directing end of life care, symptom management, medication administration in compliance with physician's orders and plan of care,
? Monitoring and interpreting symptoms and treatments and developing a plan of care in collaboration with an interdisciplinary team
? Providing education and support to patients and families
? Keeping case manager informed about any changes in a timely manner
2. Responsible for admissions and nursing visits as assigned. This includes:
? Initiating the plan of care and priority setting to ensure effective and efficient patient care deliver and achievement.
? Consulting and coordinating with the interdisciplinary team members to assess, plan, implement and evaluate patient's plan of care.
? Following a plan of ca re that assures the patient and family needs are identified, prioritized, and met in order to achieve desired clinical outcomes/goals.
? Regularly reevaluating the patient's nursing needs and making the necessary revisions in the plan of care.
3. Assuring that care, educational and end of life planning needs of patients are met.
ï?¶ Teaching patients and families disease progression, medical, social, spiritual and psychological care as well as medication review.
ï?¶ Monitoring ongoing patient care, through care conferencing and review of documentation, to determine educational and palliative care needs.
ï?¶ Utilizing knowledge of patient's age and cultural diversity in the provision of patient care and education needs.
ï?¶ Evaluating outcomes to look for decline and any discrepancies
ï?¶ Completing weekly assigned nursing assessments and documenting the supervision of the Certified Home Health Aides and Licensed Practical Nurses
ï? ¶ Collaborating with physicians to ensure accuracy of medication administration per MAR
ï?¶ Following the MAR and documenting any effect of PRN medications
ï?¶ Providing GIP level of care services and appropriate documentation to ensure that symptoms are managed.
ï?¶ Ensuring that medications are ordered and supply is adequate
ï?¶ Coordinating with volunteers to provide support and direction
ï?¶ Reporting any incidents through the RL6 reporting system and notifying supervisor, physician, and family.
4. Directs and supervises Licensed Practical Nurses (LPN) and Certified Home Health Aides (CHHA).
5. Communicates a report to the appropriate team members for the continuation of care, including reporting any changes in patient's condition to the physician and family as needed.
6. Demonstrates the knowledge and skills necessary to provide care appropriate to patients of all age groups?must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patient's needs.
7. Documents care provided for the patient encounter in the electronic medical records (per documentation standards) in a timely manner. Must document/chart observations, activities and other data related to the clinical condition of the patient and palliative care.
8. Maintains universal precautions guidelines and safety measures according to RiverStone Health policies and procedures
9. Experienced in end of live care, symptom management and control, quality and reimbursement measures for Hospice outcomes.
10. Contributes to the provision of quality process through performance improvement techniques that demonstrates positive outcomes in patient care. Receives and works to resolve patient/family concerns/complaints.
11. Carry pager and answer call lights in a time ly manner
12. Provides hygiene/personal care to patients.
13. Works collaboratively with staff and volunteers to keep the hospice home clean, tidy and welcoming following the weekly/monthly assignment designations.
14. Able to prepare meal as directed by dietician menu planning or as patient requests.
15. Facilitates and models team work.
16. Maintain knowledge of and compliance with current Medicare/Medicaid, state regulations and rules, Health Insurance Portability and Accountability Act (HIPAA) compliance and accreditation regulations for hospice services.
17. Maintain and enhance professional growth and development through attendance in orientation, continuing educational offerings as well as organizational educational programs as requested.
18. Demonstrates interpersonal, cognitive, and technical competencies and standards of care consistent with the program's and organization's mission, values and services.
19. Ability to help cover need s in hospice, hospice facilities and home care visits as assigned.
QUALIFICATIONS to perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND EXPERIENCE
1. Graduate from an accredited school with an RN degree.
2. Proficient oral and written communication skills aimed at a variety of patients, families and health care team members.
3. Knowledge of nursing process and hospice and palliative care regulations preferred.
4. Required proficiency with personal computer and keyboarding essential and be willing to learn Hospice documentation software applications including the electronic medical record.
CERTIFICATES, LICENSES, REGISTRATIONS:
1. Montana State licen sure as a Registered Nurse
2. Current CPR certification.
3. Valid Montana Driver's License in good standing with an automobile that is insured in accordance with the State of Montana.
