In this position, the Bariatric Nurse Educator/Surgery Program Coordinator is responsible for assisting patients through all phases of bariatic surgery an! d care by coordinating a system-wide delivery of service expectations including individual assessment, planning, implementation and follow-up as well as facilitating general education and resource support.

Essential Duties and Responsibilities

Program Management and Development

Acts as a liaison between all involved parties to coordinate treatment goals and established criteria in the most cost-effective way as well as following appropriate clinical pathways for care of the bariatric patient

Collaborates with physicians and hospital administration to ensure consistency in planning and designing services, integrating and coordinating services and improving organizational performance.

Maintains bariatric database at the clinic site and facilitates its use and maintenance in other program locations.

Assists in developing and ensuring the development of clinical pathways and policies and procedures in the areas of education,! surgery and aftercare.

Develops educational modules ! for quarterly bariatric in-services training for hospital and clinic staffs and provides training for new employees

Participates in marketing activities and utilizes the telemedicine network to promote the surgical program throughout the region

Care Coordination

Coordinates all elements of the surgical patient’s experience, providing optimal care and coordination of resources prior to, during and after surgical intervention including ensuring an efficient admission, communicating specialized needs and developing clinical pathways

Assures patient meets criteria for acceptance into the surgery program, arranging and receiving the established assessment and evaluations required.

Monitors patient’s progress including physical status, weight loss, nutritional adequacy and complications.

Develops and participates in a 24 hour telephone consultation service to patients for post surgical complications and support.

I! nterviews patients and develops plan for obtaining funds for surgical fees through a variety of insurance, private pay and financial assistance methods and assists patients in application processes.

Works with insurance case managers to ensure pre-certification and benefits are verified prior to surgery and also collaborates with payers and SVH staff to secure funding for patients in need of financial assistance.

Oversees the maintenance of the clothing exchange by volunteers, participants &/or clinic staff

Provides care, support, and education to patient and family throughout the in-patient perioperative and postoperative course

Provides ongoing support and education to hospital staff and is an available resource for staff during the patients perioperative and postoperative course

Patient Education

Develops and leads informational seminars, develops appropriate patient education materials and updates and maintains suppo! rt groups

Provides education, guidance and recommendations to ! patient regarding physical and emotional aspects of surgery and life after surgery and monitors patient outcomes for up to 5 years including counseling/dietary assistance

Education, Experience and Licensure/Certification Requirements

Requirements are

representative of the minimum level of knowledge, skills and/or abilities necessary to perform the essential functions of the position

Current MT RN licensure

Current BLS certification

Minimum of three years experience in a hospital or related health care setting

Ability to communicate effectively and diplomatically within a multi-functional team

Strong organizational skills and attention to detail

Ability to successfully function in a fast paced, service oriented environment

Experience in understanding and usage of computers, including the Microsoft Office suite, as well as the ability to learn applications relevant to the position
