Fort Harrison (Helena) is a 48-bed acute care, medical-surgical facility that offers a broad range of acute, chronic, and specialized inpatient and outpatient services for both male and female veterans. Specialty care includes internal medicine, gerontology, neurology, dermatology, cardiology, palliative care, pain management, medical oncology, surgery (general, vascular, laparoscopic, endoscopic), urology, orthopedics, plastic, ophthalmology, ENT, podiatry, gynecology, chiropractic care, psychiatry (including outpatient substance abuse treatment and PTSD and MST specific care), and ambulatory care (primary care). Radiology Service provides a broad range of diagnostic and interventional care provided on a full time basis. P! athology Services are available on site also provided on a ful! l time basis. Telemedicine services are available for psychiatry, radiology, gynecology, primary care, ophthalmology and tele-home health. The 24-bed inpatient mental health facility serves patients needing treatment for PTSD, Substance Abuse and Acute Care.
A 30-bed Community Living Center (CLC) that provides general and ventilator dependent care is located in Miles City. Primary care is provided at Anaconda, Billings, Bozeman, Cutbank, Glasgow, Glendive, Great Falls, Havre, Kalispell, Lewistown, Miles City, and Missoula. Primary Care Telehealth Outreach Clinics are located in Hamilton and Plentywood. Staff and contract specialists visit the clinics regularly and provide access to specialty care as needed.

DUTIES:
The LPN is responsible for observing, recording, and reporting changes in the patientÂ's condition, as well as assisting in the development and revision of the patient-driven, holistic care plan for life. The LPN is a member of the larger team. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies, and procedures. The LPN demonstrates active collaboration and communication with the teamle! t and the larger team.
1. Reviews daily schedules for patient care needs.
2. Medication reconciliation.
3. Rooming patients and doing vital signs.
4. Performing appropriate clinical reminders if not previously done in pre-visit call.
5. Completes orders per clinical or protocol.
6. Oxygen therapy.
7. Medication administration.
8. Clinical reminders with appropriate consults and alerts as indicated.
9. Check expiration dates of meds and supplies.
10. Delegated procedures and treatments.
11. Documentation.
12. Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
13. Keeps clinician on schedule.
14. Ensures visit closure.
15. Adheres to standards of practice and safe technique in preparation and administration of
m! edication.
16. Demonstrates understanding of age-and populatio! n-specific characteristics that affect the health care needs of the Veteran.
17. Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes as directed when appropriate.
18. Based on the patientÂ's goals, provides patient and family health education, to include self-management, prevention, and wellness. Reinforces goal set by patient.
19. Interacts and communicates appropriately with patients and family members.
20. Demonstrates professional behavior, effective communication, and interpersonal skills.
21. Adheres to principles of infection control, safety, and universal precautions.
22. Incumbent possesses knowledge of proper handling and pre-cleaning of Reusable Medical Equipment. (RME)
23. Demonstrates competence utilizing special equipment needed to perform job duties.
24. Follows policy and procedures related to fire and disaster drills, safety, and ! security measures.
25. Demonstrates initiative in assisting the teamlet and larger teams.
26. Assists in the orientation and competency validation of students, peers, and clerical staff.
27. Practices sound ethical principles that supports diversity, maintains confidentiality of patient information, and protects patient/caregiver rights.
28. Completes mandatory reviewing, in order to promote patient self management toward individualized life plan of care. Acts as health coach to patients, ensuring patient understanding and agreement with plan of care.
29. Supports patient-driven care in a safe, efficient and cost-effective manner, using evidence-based principles.
30. Participates in daily huddle, weekly team meetings.
31. Performs pre-appointment phone calls, reminders, and reviews.
32. Assists with orientation: PCMH patient orientation and new patient orientation.
33. Gathers informatio! n for RN Care Manager to assists with triage, messages from patients an! d face-to-face visits.
34. Arranges follow-up.
35. Identifies additional services needed.
QUALIFICATIONS REQUIRED:
BASIC REQUIREMENTS:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Licensure. Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or commonwealth (i.e., Puerto Rico) of the United States or District of Columbia.
English Language Proficiency
PREFERRED EXPERIENCE:
Preferred experience working in specialty clinics such as orthopedic, neurology, or cardiac.

GRADE DETERMINATIONS:
Grade Determination.

GS-3.
None beyond the basic requirements.
[LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.]
GS-4.
Six months of qualifying experience as an LPN or LVN; or
Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or
Graduation from an approved school of at least 24 months ! duration.
GS-5
Completion of at least 1 year of quali! fying experience at the GS-4 level or equivalent.
Demonstrated knowledge and ability to provide a full range of practical nursing care to patients with a variety of physical and/or behavioral problems.
Knowledge and skill sufficient to prepare, administer, and appropriately document actions taken specific to commonly prescribed oral, topical, subcutaneous, intramuscular, and/or intravenous medications as permitted by approved local facility policies and procedures. Observation and documentation will include patient's response to medication administered and the reporting of any noted change in patient's condition to Rn or MD/DO.
Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards, and supply deficiencies, promptly reporting to appropriate personnel for corrective action
Completed work is under the general supervision of an RN or MD/DO.
Individuals at this grade level are expected to h! ave a broad working knowledge of practical nursing procedures.
However, completion of more complex practices or procedures may be subject to closer higher-level review.
GS-6

Technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations.
Knowledge and ability to appropriately carry out assigned patient care based on the patientsÂ' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information.

Ability to observe, identify and respond to the patientÂ! 's needs for [care, including] medication, equipment-assisted care and [patient/family education].

Establishes constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills.
Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery.
Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers.
Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patientÂ's condition befor! e, during, and following the procedure.
Serves as a preceptor ! in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures.
Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
