Provide personal care, support, and training to elderly, disabled, or terminally ill patients and their care-givers in a hospice home setting. Primary duties include meal preparation, bathing, dressing, and maintaining hygiene of patients, light housekeeping, patient observations, communicating patient changes to nursing staff, working with and relieving the patient’s primary care taker. ESSENTIAL KNOWLEDGE, SKILLS AND RESPONSIBILITIES include the following. Other duties as assigned.
1. Performs personal grooming care of patients, including bathing, hair care! , manicure, pedicure, and oral care
2. Performs light hous! ekeeping tasks, including changing bed linens, laundry, washing dishes, and light meal preparation
3. Performs minor health care procedures as assigned, including bed bathing, applying lotion, turning and positioning patients
4. Assisting to administer medications that are ordinarily self-administered when assigned.
5. Supports patients with range of motion, ambulation and exercise as directed by nursing or therapy personal as per the plan of care.
6. Communicates with and supports caregiver regarding the patient’s condition and relieves caregiver if required
7. Records observations of the patient’s condition and needs and reports any changes in patient’s medical, nursing, rehabilitative, and social changes in condition or family situation to appropriate team member and supervisor
8. Documents care provided or refused by the patient in the electronic medical records in a timely manner. Must document/chart observations, activities and other data! related to the personal cares of the patient.
9. Attends meetings, conferences, workshops, and training sessions and reviews publications and audio-visual materials to become and remain current on principles, practices, and new developments in assigned work areas
10. Maintains universal precautions guidelines and safety measures according to RiverStone Health policies and procedures
11. Maintain knowledge in end of live care and measures for Hospice care.
12. Able to prepare meals for larger numbers per food safety guild lines when working at hospice facility
13. Substantial knowledge of first aid and the ability to administer simple medical treatments
14. Substantial knowledge of body mechanics and physical manipulation, repositioning, and transfer of the patient
15. Ability to communicate effectively with others, both orally and in writing, using both technical and non-technical language
16. Ability to understand and follow oral and/or wri! tten policies, procedures, and instructions
17. Demonstrates the kn! owledge and skills necessary to provide care appropriate to patients of all age groups.
18. Maintain knowledge of and compliance with current regulations and rules, Health Insurance Portability and Accountability Act (HIPAA) compliance and accreditation regulations for hospice services.
19. Keeps immediate supervisor and designated others accurately informed concerning work progress, including present and potential work problems and contributes to the provision of quality process through suggestions for new or improved ways of addressing such problems.
20. Ability to perform a wide variety of duties and responsibilities with accuracy and speed under the pressure of time-sensitive deadlines
21. Ability and willingness to quickly learn and put to use new skills and knowledge brought about by rapidly changing information and/or technology;
22. Ability to help cover needs in hospice, hospice facilities and home care visits as assigned.
QUALIFICATIONS to ! perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION AND EXPERIENCE
High School Diploma or GED; and
Some experience in patient care; or
Any combination of experience and training which provides the equivalent scope of knowledge, skills, and abilities necessary to perform the work.

CERTIFICATES, LICENSES, REGISTRATIONS :
1. Current certification as a Nursing Assistant and Home Health Aide in the State of Montana;
OR Current Montana CNA certification
2. Valid State of Montana Driver’s License in good standing, appropriate vehicle insurance, and a vehicle in good working condition.
